This was also the first time I've seen it.
The zombies somehow look more like ghosts than zombies.

Likely an artist who was pretty good at art but had little direction to go with.

May be the artist never played the game.
(lot of cases like this back then,
they're only given very few photos about the games (screenshots) and asked to draw based on that).



Or may be because of time retraints, just like the famous Megaman cover.
